- Please contact the nearest office of Star. The office can be located from Office Locator from our website.
- You will be required to submit the following,
- a. Proposal form duly filled and signed.
- b. Portability form to be duly filled and signed.
- c. Passport size photographs of persons to be covered.
- d. Policy copies of the previous insurance policies.
- e. Pre Insurance medical examination reports, wherever, applicable. Medical examination will be done at our cost.
- In respect of proposals received for coverage of persons upto 45 years, you may please approach Star Office atleast 7 days prior to renewal date.
In respect of proposals received for coverage of persons above 45 years, you may please approach Star office atleast 30 days prior to renewal date.
