8. Enter the Intimator's Name
9. Enter your Relationship with the patient
10. Enter the Attendee's Mobile Number
11. Enter the Name of the Proposer
12. Enter the Insured Patient's Name
13. Enter the Insured's Email ID
14. Enter the Insured's Contact Number
15. Enter the Date of Accident
16. Enter the Place of Accident
17. Enter the FIR Details
18. Enter the Reason for Admission
19. Enter the Hospital Admission Date
20. Select your Hospital Type
21. Enter the Pincode of the Hospital
22. Click Hospital List
23. Select the Hospital from the search list
24. Review your details > Click Confirm
25. You have Successfully Intimated your claim
