Healthcare professionals use BMI or Body Mass Index calculators to assess an individual’s body weight for height. These calculators help to determine if someone is underweight, normal, overweight or obese based on a standardised formula.
Though being a widely used screening tool, there are certain limitations to it, especially for women. A common question that arises is whether BMI calculators for women consider age and hormonal fluctuations. Usually, the calculation of BMI fails to take these factors into account. Consequently, this oversight makes this measurement less accurate for women.

The formula for BMI is kg/(m)2. While this simple formula is a quick way to screen your body composition, it fails to consider the unique shape and size of each body. Here is a breakdown of the weight status of women based on BMI levels:
● Underweight: Less than 18.5
● Normal Weight: 18.5 to 24.9
● Overweight: Between 25 and 29.9
● Obese: More than 30
Body composition undergoes significant changes with age. This is a universal rule. Interestingly, not all bodily changes follow the same direction. For example, muscle tends to decrease with age for women, while fat mass often increases, especially after menopause.
Postmenopausal women also have higher chances of developing abdominal fat. This shift in body composition results in a higher body fat percentage. The inflation of fat occurs even if a woman’s weight remains stable.
Since the calculation of the Body Mass Index fails to differentiate between fat and lean body mass, such as muscle and bones, it can misinterpret the results. It classifies all as an increased weight without considering the distribution. This is why most BMI results show older women as overweight or obese, even if they are not.

Hormones play a significant role in determining body composition and fat distribution in women. A woman’s body goes through massive hormonal shifts during menstruation, puberty, menopause and pregnancy. Increased levels of progesterone during the menstrual cycle promote the water retention ability of the body. This feeling of swelling often leads to temporary weight gain, which can result in an inaccurate BMI reading.
Similarly, during menopause, there is a sharp decline in oestrogen levels, which promotes fat accumulation in the abdominal area, resulting in a higher BMI. However, this does not necessarily indicate obesity.

Though being an easy method to screen weight categories, BMI is not always an accurate measure of body fat for women. This is particularly true for those who are older or going through hormonal alterations. BMI fails to distinguish between fat and muscle, leading to miscalculation.
Women with higher muscle mass, such as athletes, can have a high BMI but still have low body fat. This means they are healthy, regardless of what their BMI suggests. Conversely, BMI calculation may be normal for women with lower muscle mass and higher body fat. However, they are at higher risk for health issues due to excess fat.
As commonly used, BMI calculators for women do not consider age, hormonal changes, or other important factors that influence body composition. Thus, BMI should not be relied upon as the sole indicator of health, particularly for women.
