Group 1: Government Employees and Pensioners

  • The individual should be either a serving government employee or a retired employee (pensioner).

  • All indigenous and permanent residents of Nagaland who are not covered under Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (AB-PMJAY) or any other government-funded health insurance scheme.

What are the Mandatory Documents for the Nagaland Health Insurance Scheme?

 

The documents required for online registration will not be the same for each category. Some documents, according to the group, include:

 

General Public (CMHIS-GEN)

 

  • An active mobile phone number.
  • Permanent Residential Certificate (PRC) or Indigenous Inhabitant Certificate (IIC) issued after 2016.
  • Ration Card (for eligible households, eCards may be generated through the BIS portal)

 

Employees without a PIMS Number

 

  • An active mobile phone number
  • Government-issued ID card
  • Appointment or engagement order
  • Proof of employment continuity
  • Completed registration form signed by the Village Council Chairman or Ward Chairman.

 

Employee and Pensioner Category (CMHIS-EP)

 

  • Dependent Family Members
  • Proof of relationship and date of birth (such as Aadhaar card, birth certificate, etc)
  • Disability certificate if applicable

 

Government Employees

 

  • An active mobile phone number
  • PIMS code.

 

Pensioners

 

  • An active mobile phone number.
  • The first three pages of the Pensioner’s PPO book. These pages should clearly show pay scale, designation, PPO number, and last salary details.

 

How to Enrol in CMHIS Nagaland?

 

Enrolling in CMHIS Nagaland is easy. You can follow the steps listed below to complete the registration process.

 

  • Step 1: Go to the official website of the Chief Minister’s Health Insurance Scheme, Nagaland.
  • Step 2: Select the ‘Registration’ option available on the screen.
  • Step 3: Fill in all the required information, such as your name, age, family information, income, and past medical history, if any. Ensure all the information provided is accurate, as you may have to face issues later.
  • Step 4: Upload all the necessary documents. In addition, you can also visit the nearest Common Service Centre (CSC) in your area for any help in the registration process.
  • Step 5: Now, the common service centre will cross-check all the information provided by you. Once verified, they will upload the information to the official CMHIS online portal for processing.
  • Step 6: The insurance provider will review the application. After reviewing, the authority will decide whether to approve or reject your application form.
  • Step 7: If the insurance provider approves your application, you will receive a confirmation message on your registered email ID or mobile number.
