  • Over 600 medical procedures are covered, such as treatments for cardiovascular, neurological, and renal diseases, cancer, burns, neonatal diseases, hip and knee replacement, and major transplants.
  • Each beneficiary is issued a QR-coded card containing biometric data, a unique registration number (URN), a photograph, and district and taluka details.
  • The scheme even reimburses up to ₹300 per hospitalization for travel expenses.
  • It also covers the cost of transporting mortal remains at ₹6 per km from the hospital to the beneficiary's home.
  • All benefits under the PMJAY Scheme are included in this program.

What are the Exclusions Under the MA Yojana?


While the Mukhyamantri Amrutum Vatsalya covers several treatments, it does not include:

Eligibility for MA Scheme


●  BPL


Individuals and families listed in the BPL list by the State Government are eligible for the MA Yojana. All family members must be present during enrollment.


●  APL


For families just above the poverty line (APL), eligibility for the MA Vatsalya Yojana is possible if they meet the following criteria:


●  Annual family income up to ₹4,00,000.
●  Family members who are Accredited Social Health Activist (ASHA) workers.
●  Family members who are accredited reporters.
●  Class-3 and Class-4 employees who are on a fixed payroll by the State Government.
●  Senior citizens with a total family income up to ₹6,00,000. 

 


How to Enroll under MA Card Yojana?


Only eligible families can enrol and receive benefits from this scheme. Here are some easy steps to enrol under Mukhyamantri Amrutum:

 

  • Government authorities (Taluka and Civic Centers) maintain a list of families with an annual income under ₹4 Lakh, which is also available on the MA Gujarat website.
  • To enroll, applicants need to visit Civic Centers or Taluka kiosks, with mobile kiosks available at different locations.
  • Biometric information, including fingerprints, iris scans, and family photos, will be captured at the kiosks.
  • Applicants must fill out an application form in Gujarati with family details.
  • After document verification, an MA card and a pamphlet with details of the scheme and empanelled hospitals will be provided.
  • Once the documents are given, the enrollment process is complete.


Documents Required to Apply for MA Scheme


Eligible applicants are required to provide the following documents when visiting the Nodal agency:

 

  • ID Proof (e.g., Driving License/Aadhaar card/PAN Card/Voter ID Card)
  • Address Proof (e.g., Ration card)
  • Income Proof

  • Other required documents (if applicable) 


Specific Details of MA Scheme Health Card

 

  1. Mobile apps provide information on how to apply for or download the MA Scheme health card, but the person must be present at the kiosk for enrollment.
  2. At the enrollment centre, a person will receive the MA Vatsalya card application form (PDF or physical copy) to fill in their details.
  3. After enrollment, they can revisit the kiosk to check the status of their MA Vatsalya card.
  4. In case the beneficiaries have internet access, they can even log in to the MA Gujarat website to check the MA card status and learn more about the scheme.
  5. Beneficiaries are required to present the MA Yojana card during medical emergencies to access coverage.


How to Claim the MA Scheme?


The claim process for Mukhyamantri Amrutum is simple and smooth, with no documentation requirements. Here are the steps to follow:

 

  • Beneficiaries only need to submit their QR card at a network hospital to start the verification process.
  • After quick verification and Mukhyamantri Amrutum Vatsalya Card check, treatment can start immediately, and the claims are cashless, easing the financial burden on the patient and family.
  • Once treatment is complete, the hospital sends the details to the insurance company, and the claim is settled directly between the hospital and the insurer.
